Follow the steps below to get the process started
Step 1
Get a Diagnosis &
Diagnostic Report
An Autism-Related diagnosis* is required by insurances to begin ABA services.
*Banner health insurance does not require a diagnosis, but does require participants to be enrolled in Health Home for ASD services.
All families: please obtain a diagnostic report.
Step 2
Obtain a Referral
A referral is required for all children being funded by a private insurance plan other than Banner Health Insurance.
Banner Insurance
Banner health insurance does not require a diagnosis, but does require participants to be enrolled in Health Home for ASD services.
Private Insurance
Need a script from PCP and diagnostic reports with Autism diagnosis clearly stated to request authorizaton. You will need a prior authorization on file for services.
Arizona ESA Scholarship
Requires a 504 or IEP/MET on file with AZ ESA and category of special needs student to be eligible for ABA therapy services paid with ESA funds. Universal students are not eligible to use ESA funds for ABA therapy.

Contact Strong Beginnings
You can contact us here on the website by using the contact box below or you can call our office to discuss availability and scheduling before submitting request.
Once process is started, intake packet may be mailed out to pre-fill before assessment.
Step 5
Schedule Services
Scheduling for services will be based upon family and provider availability.
Once assessment has been completed, recommended hours and scheduling will be finalized.
